People use different programs like Daz, Poser, 3D Max and so on. I use Gmod which is kind of a underdog to these professional platforms, but I like using it nevertheless.
What you in principle do is posing models in the desired position, add lights, build scenery and add effects, depending on the picture in mind.
And making these models are far from easy. It takes time and a lot of learning before you are close to a decent model.
Take me! I know how to pose, but I do not know squat about making models. I have buddies who do that.

Yeah, the model I was just using is actually a standard variation of Stephanie 4 from DAZ3D. That said, the skin and hair are from other commercial vendors, and her facial expressions use another DAZ3D product.

It can get pretty involved, but there are some nice free resources online.
